Henna Art Consent and Release of Liability

By signing this document, I acknowledge and agree to the following terms regarding the temporary henna body art provided by Henna Spark:

1. Ingredients & Allergy Acknowledgment: I understand that Henna Spark uses a 100% natural henna paste containing organic henna powder, lemon juice, sugar, and pure essential oils. I confirm that I do not have any known allergies to these ingredients, nor do I have G6PD deficiency, which can cause adverse reactions to henna.

2. FDA Disclaimer: I understand that while Henna Spark’s ingredients are natural and plant-based, the U.S. Food and Drug Administration (FDA) has only approved henna for use as a hair dye; it is not currently FDA-approved for direct application to the skin for body art. I am choosing to receive this temporary body art at my own discretion and risk.

3. Assumption of Risk & Release of Liability: I accept full responsibility for any skin reactions, allergies, rashes, or medical issues that may occur as a result of the henna application. I hereby completely release, indemnify, and hold harmless Henna Spark, its artists, and the event organizers from any and all claims, liabilities, or damages arising from this service.

4. Care & Maintenance: I understand that the longevity and color of the henna stain depend on my individual skin chemistry and how well I follow the provided aftercare instructions. Henna Spark does not guarantee the final color or lifespan of the design.

5. Minors (If Applicable): If the client receiving the henna is under the age of 18, I confirm that I am the parent or legal guardian and I consent to the application of henna under these terms.
